Objectives: This study attempts to analyse the ethical dilemmas arising from the microallocation of scarce health care resources, in terms of deontology and utilitarianism.Methods: A group of 395 people were interviewed in the region of Diadema, greater San Paulo, Brazil, while visiting patients in the only state hospital in town. Each interviewee was given a list of eight simulated emergencies . In each of the eight cases the interviewee had to choose which of the two patients described, both of whom (...) suffered from exactly the same problem, should receive the only hospital bed currently available. The differences between the hypothetical patients were as follows: age, gender, family dependency, and lifestyle. Each interviewee was asked to justify one of their responses. These responses were then analysed.Results: The results pointed to the co-existence of deontological and utilitarian orientations among the people interviewed. A tendency to give priority to the destitute was revealed throughout the research, contradicting the idea that society, valuing only productive people, wishes only such people to receive the most resources, thus maximising the benefits to be gained from resources. The results showed that people’s disapproval of the alcoholic was stronger than that of the nicotine abuser. (shrink)

The author holds that the enduring achievement of the modern mind is the recognition of a sharp distinction between fact and value; this work is a history of that distinction. In separate sections devoted to the history of scientific method and the history of value theory, Hall covers the ground from the medieval period to the present. His conclusion strikes a pessimistic note; modernity, after distinguishing fact and value, has had marvelous success with the former but is in danger of (...) losing the latter altogether. Towards resolution of this difficulty, he suggests, with tantalizing brevity and obscurity, that though value statements may be emotive, we may perhaps regard emotion as a genuinely cognitive state somehow putting us in touch with real features of the world. While he writes with a pleasant down-to-earth directness, Hall never makes clear precisely what this basic cleavage between fact and value amounts to. And his history, though illuminating, is not always well-balanced; Kant as a value theorist, gets less space than either Locke or Machiavelli and only a third as much as Hobbes. ABSTRACT: René Descartes' Cogito is an example of a paradigmatic thought experiment, herald of both subjectivism and new science in Europe's Modern Age, that seems to have escaped the attention of thought experiment philosophers. On deep analysis, the Cogito appears as universal instantiation. The Cogito has strong rhetorical effects for it narratively generalizes from I to all human kind, and its historical and philosophical success can be explained from its concise enthymematic structure that rings true in many possible senses. We (...) consider it a preeminent example of a thought experiment as it states the power of thinking as its very contents. From Descartes' methodology of doubt we can conclude that, e.g., on a Wittgensteinian interpretation, the Cogito is a logical thought experiment rather than a psychological one. RESUMO: O Cogito de René Descartes é um exemplo de experimento mental paradigmático, precursor tanto do subjetivismo quanto da nova ciência, na Idade Moderna europeia, o qual parece ter escapado à atenção dos filósofos que estudaram o experimento mental. Na análise profunda, o Cogito aparece como uma instanciação universal. O Cogito tem fortes efeitos retóricos por si mesmo, generalizando narrativamente desde o eu para toda a espécie humana, e seu sucesso histórico e filosófico pode ser explicado por sua estrutura entimemática concisa, que soa através de muitos sentidos possíveis. Consideramos que é um exemplo proeminente de um experimento mental, na medida em que afirma o poder de pensar como seus próprios conteúdos. A partir da metodologia da dúvida de Descartes, podemos concluir que, em uma interpretação wittgensteiniana, o Cogito é um experimento mental mais lógico que psicológico. (shrink)

Objectives To investigate empirically the motivations for not consenting to DNA biobanking in a Swedish population-based study and to discuss the implications. Design Structured questionnaires and semistructured interviews. Setting A longitudinal epidemiological project (PART) ongoing since 1998 in Stockholm, Sweden. The DNA-collection wave took place during 2006–7. Participants 903 individuals completed the questionnaire (participation rate 36%) and 23 were interviewed. All individuals had participated in both non-genetic waves of the project, but refused to contribute saliva samples during the DNA-collection wave. (...) Main outcome measures Motivations behind refusing to consent to DNA biobanking, with subsequent focus on participants' explanations regarding this unwillingness. Results Public refusal to consent to DNA biobanking, as revealed by the questionnaire, was mainly explained by a lack of personal relevance of DNA contribution and feelings of discomfort related to the DNA being used for purposes other than the respective study. Interviews of individuals representing the second motivation, revealed a significant mistrust of DNA biobank studies. The underlying beliefs and attitudes were associated with concerns about integrity, privacy, suspiciousness and insecurity. However, most interviewees were supportive of genetic research per se and interpreted their mistrust in the light of distressing environmental influences. Conclusion The results suggest a need for guidelines on benefit sharing, as well as trustworthy and stable measures to maintain privacy, as a means for increasing personal relevance and trust among potential participants in genetic research. Measures taken from biobanks seem insufficient in maintaining and increasing trust, suggesting that broader societal measures should be taken. (shrink)

BackgroundAs moral case deliberations (MCDs) have increasingly been implemented in health care institutions as a form of ethics support, it is relevant to know whether and how MCDs actually contribute to positive changes in care. Insight is needed on what actually happens in daily care practice following MCD sessions. This study aimed at investigating the impact of MCD and exploring how ‘impact of MCD’ should be conceptualized for future research.MethodsA multiple-case study was conducted in a care organization for people with (...) intellectual disabilities and/or acquired brain injury, by observing MCD sessions as ‘cases’, followed by interviews with health care professionals concerning the follow-up to these cases, and a focus group with involved MCD facilitators. A conceptual scheme concerning the possible impact formed the basis for analysis: (1) individual moral awareness; (2) the actions of health care professionals; (3) collaboration among health care professionals; (4) the concrete situation of the client; (5) the client’s quality of care and life; (6) the organizational and policy level.ResultsAccording to interviewees, their moral awareness and their collaboration, both among colleagues and with clients’ relatives, improved after MCD. Perceived impact on client situation, quality of care/life and the organizational level varied among interviewees or was difficult to define or link to MCD. Three aspects were added to the conceptual scheme concerning the impact of MCD: (a) preparations and expectations prior to the MCD session; (b) a translational step between the conclusions of the MCD session and practical events in the following period, and (c) collaboration with clients’ relatives. A negative impact of MCD was also found on misunderstandings among participants and disappointment about lack of follow-up.ConclusionsConcretizing and conceptualizing the ‘impact’ of MCD is complicated as many factors play a role either before or during the transition from MCD to practice. It is important to consider ‘impact’ in a broad sense and to relate it to the goals and context of the MCD in question. Future studies in this field should pay additional attention to the preparations, content and process involved in ethics support, including clients’ and relatives’ experiences. (shrink)

Among them is a fascinating essay showing that Jonathan Edwards consciously developed his homilectical methods in terms of Locke's psychology and epistemology. These pieces constitute "a rank of spotlights on the massive narrative of the movement of the European culture into the vacant wilderness of America"; each is prefaced by a newly written introduction indicating its relevance to the unifying theme of the volume, viz., (...) "the obsessive American drama: nature vs. civilization." It is this attempt to civilize nature that constitutes the "errand into the wilderness." The volume is dominated throughout by Miller's conviction that intellectual history is as crucial to the understanding of a nation or a culture as is social or economic history; and the illumination which the essays actually afford is compelling evidence for his conviction.--A. The thesis of this essay in philosophical analysis is that ethical words are used referentially, that for the most part they have a single unitary sense, and that they refer to whatever is "happy-making" or satisfying. The author supports this conclusion by means of detailed refutations of some of the criticisms brought against naturalism, paying special attention to the "naturalistic fallacy" argument as developed by Moore, Ewing and the contemporary non-cognitivists. He concludes that philosophical analysis ought to reject the "method (...) of introspection" in favor of inductive inquiries into the ways in which as a matter of fact people use ethical words.--A.
